IN CONSIDERATION OF the covenants and agreements contained in this Agreement and other good and valuable consideration, the receipt of which is hereby acknowledged, the parties to this Agreement agree as follows:

Consideration

1.              Being of lawful age and in consideration of being permitted to participate in the activity described below, the Participant releases and forever discharges the Activity Provider, its owners, directors, officers, employees, agents, assigns, legal representatives and successors from all manner of actions, causes of action, debts, accounts, bonds, contracts, claims and demands for or by reason of any injury to person or property, including injury resulting in the death of the Participant, which has been or may be sustained as a consequence of the Participant's participation in the activity described below, and not withstanding that such damage, loss or injury may have been caused solely or partly by the negligence of the Activity Provider.

2.              The Participant understands that the Participant would not be permitted to participate in the activity described below unless the Participant signed this Agreement.

3.              The Participant will participate in the following activity: Flagler Sheriff's PAL, Inc. Sports and Activities Programs.

Concurrent Release

4.              The Participant acknowledges that this Agreement is given with the express intention of effecting the extinguishment of certain obligations owed to the Participant and with the intention of binding the Participant's spouse, heirs, executors, administrators, legal representatives and assigns.

Fitness to Participate

5.              The Participant acknowledges that the Participant does not have any physical limitations, medical ailments, physical or mental disabilities that would limit or prevent the Participant from participating in the above mentioned activity. If required, the Participant will obtain a medical examination and clearance.

Full and Final Settlement

6.              The Participant hereby acknowledges and agrees that the Participant has carefully read this Agreement, that the Participant fully understands the same, and that the Participant is freely and voluntarily executing the same.

7.              The Participant understands that by signing this Agreement, the Participant agrees to be forever prevented from suing or otherwise claiming against the Activity Provider for any property loss or personal injury that the Participant may sustain while participating in or preparing for the above noted activity.

8.              The Participant has been given the opportunity and has been encouraged to seek independent legal advice prior to signing this Agreement.

 9.              This Agreement contains the entire agreement between the parties to this Agreement and the terms of this Agreement are contractual and not a mere recital.

Governing Law

10.           This Agreement will be governed by and construed in accordance with the laws of the State of Florida.

NOTICE TO THE MINOR CHILD’S NATURAL GUARDIAN

READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF FLAGLERSHERIFFS PAL, INC. USES REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD’S RIGHT AND YOUR RIGHT TO RECOVER FROM FLAGLERSHERIFFS PAL, INC. IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND FLAGLERSHERIFFS PAL, INC. HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN THIS FORM.

If a waiver or release complies with paragraph and waives no more than allowed under this subsection, there is a rebuttable presumption that the waiver or release is valid and that any injury or damage to the minor child arose from the inherent risk involved in the activity.

1. To rebut the presumption that the waiver or release is valid, a claimant must demonstrate by a preponderance of the evidence that the waiver or release does not comply with this subsection.

2. To rebut the presumption that the injury or damage to the minor child arose from an inherent risk involved in the activity, a claimant must demonstrate by clear and convincing evidence that the conduct, condition, or other cause resulting in the injury or damage was not an inherent risk  of the activity.

3. If a presumption under this paragraph is rebutted, liability and compensatory damages must be established by a preponderance of the evidence.

Nothing in this subsection limits the ability of natural guardians, on behalf of any of their minor children, to waive and release, in advance, any claim or cause of action against a noncommercial activity provider, or its owners, affiliates, employees, or agents, to the extent authorized by common law.

Youth Program Policies and Procedures

Updated for Current Florida Law and Best Practices

The Flagler Sheriff’s Police Athletic League (“FSPAL”) is a nonprofit 501(c)(3) youth-serving organization operating with the cooperative support of the Flagler County Sheriff’s Office.

FSPAL is committed to providing safe, structured, and inclusive programs for youth in Flagler County regardless of race, color, religion, national origin, disability, sex, or financial status. FSPAL also operates its programs in a manner consistent with Florida youth protection requirements, applicable civil rights principles, and recognized youth sports safety standards.

Mission Statement
FSPAL’s mission is to reduce juvenile crime and promote positive youth development by providing and coordinating quality programs and activities that foster character, integrity, physical and mental fitness, citizenship, sportsmanship, and personal well-being.


1. Youth Protection and Abuse Prevention Policy

1.1 Purpose

FSPAL programs are designed to help youth develop positive life skills, self-confidence, teamwork, and leadership in a safe and supportive environment.

1.2 Zero-Tolerance Standard

FSPAL maintains a zero-tolerance policy toward abuse, neglect, exploitation, harassment, bullying, hazing, retaliation, and sexual misconduct. This includes physical, emotional, verbal, electronic, and sexual misconduct.

1.3 Participant Rights

All participants have the right to:

  • Feel safe

  • Be treated with dignity and respect

  • Participate in a positive environment

  • Be protected from abuse, neglect, harassment, and exploitation

1.4 Protective Practices

FSPAL will take reasonable and proactive steps to protect youth through screening, supervision, training, reporting procedures, and enforceable conduct standards. FSPAL follows nationally recognized youth protection practices, including open and transparent communication and limiting one-on-one adult/minor interactions whenever possible.


2. Mandatory Reporting of Child Abuse

2.1 Reporting Requirement

Under Florida Statute § 39.201, any person who knows of or has reasonable cause to suspect child abuse, abandonment, or neglect must report it immediately to the Florida Abuse Hotline.

Reports may be made by:

  • Phone: 1-800-962-2873

  • Online: Florida Abuse Hotline Reporting Portal

If a child is in immediate danger, call 911 first.

2.2 Internal Notification

All FSPAL employees, volunteers, coaches, assistant coaches, team parents, and program leaders must immediately report suspected child abuse or neglect and must also notify the FSPAL Director as soon as practical after making the report, unless doing so would place a child at greater risk or interfere with law enforcement.

2.3 Non-Retaliation

No person who makes a good-faith report shall be subject to retaliation.


3. Volunteer and Coach Screening

3.1 Screening Requirement

All coaches, assistant coaches, referees, team parents, mentors, and volunteers with direct access to youth must complete FSPAL’s screening and approval process before participating in any youth program activity.

3.2 Minimum Screening Standards

At a minimum, the process will include:

  • A completed volunteer application

  • Identity verification

  • Criminal history screening appropriate to the role

  • Sex offender registry review

  • Acknowledgment of FSPAL policies

  • Approval from FSPAL administration prior to contact with youth

3.3 Statutory Compliance

For any role that qualifies as an “athletic coach” under Florida Statute § 943.0438, FSPAL will comply with the Level 2 screening and disqualification requirements of that statute according to its effective schedule. The statute currently provides for Level 2 screening of current and prospective athletic coaches effective July 1, 2026, and requires retention of screening documentation for at least five years.

3.4 Administrative Authority

FSPAL reserves the right to deny, suspend, or revoke volunteer eligibility at any time in order to protect program participants.


4. Two-Adult Rule and Supervision Policy

4.1 General Rule

Whenever reasonably possible, FSPAL will require the presence of two approved adults during youth activities, meetings, locker room supervision, transportation coordination, and other participant-facing functions.

4.2 One-on-One Interactions

Adults should not be alone with a minor in a private or non-observable setting. Any necessary one-on-one interaction must be:

  • For a legitimate program purpose

  • Observable and interruptible

  • Documented or conducted with parent or guardian awareness when feasible

4.3 Supervision Responsibility

Head coaches are responsible for ensuring participants are supervised before, during, and after practices, games, and events until released to a parent, guardian, or other authorized adult.


5. Youth Protection and Anti-Harassment Policy

5.1 Commitment

FSPAL is committed to providing a safe, respectful, and harassment-free environment for all participants, families, volunteers, and staff.

5.2 Prohibited Conduct

Prohibited conduct includes:

  • Bullying or cyberbullying

  • Verbal abuse or threats

  • Harassment or discrimination based on race, color, religion, sex, national origin, or disability

  • Sexual harassment

  • Sexual misconduct

  • Hazing

  • Stalking

  • Intimidation

  • Retaliation against any person who reports misconduct in good faith

Examples of prohibited conduct include offensive slurs, degrading comments, repeated humiliation, unwanted physical contact, coercive behavior, inappropriate jokes, sharing sexual content, or electronically targeting a participant.

5.3 Reporting

Any participant, parent, volunteer, or staff member who experiences or witnesses prohibited conduct must report it promptly to the FSPAL Director or designee. Complaints may also be submitted in writing to pal@flaglersheriff.com .

5.4 Corrective Action

FSPAL will review reports promptly, maintain confidentiality to the extent reasonably possible, and take corrective action as warranted. Disciplinary action may include a warning, suspension, removal from activities, permanent ban, and referral to law enforcement or child protection authorities where appropriate.


6. Electronic Communication Policy

6.1 Standard

Electronic communications involving minor participants must be professional, program-related, and transparent.

6.2 Permitted Methods

Permitted communication methods include:

  • Gymdesk or other approved team management platforms

  • Group text messages that include a parent or guardian

  • Team emails that include a parent or guardian

  • Emergency communications reasonably necessary for participant safety

6.3 Prohibited Conduct

Coaches, volunteers, and staff may not:

  • Privately text, direct message, or otherwise message a minor participant without including the parent or guardian or another approved adult

  • Use disappearing-message platforms for youth communications

  • Engage in personal, romantic, flirtatious, or secret communications

  • Communicate with minors late at night except in true emergencies

  • “Friend,” follow, or privately interact with minor participants through personal social media accounts

6.4 Content Standard

All communications must relate to legitimate program matters such as schedules, logistics, weather alerts, equipment needs, or participant safety.


7. Transportation Policy

7.1 Parent Responsibility

Parents and guardians are primarily responsible for transporting their children to and from practices, games, and events.

7.2 Limited Exception

FSPAL strongly discourages coaches, volunteers, or staff from transporting participants unless the transportation is:

  • Necessary

  • Pre-approved by FSPAL administration

  • Authorized in writing by the parent or guardian

7.3 Driver Requirements

If approved transportation is provided by a coach, volunteer, or staff member, the driver must:

  • Be an approved FSPAL volunteer or employee

  • Possess a valid driver’s license

  • Maintain current automobile insurance meeting Florida legal requirements

  • Operate a registered and safe vehicle

  • Comply with all traffic laws

  • Require seat belt use by all occupants

  • Avoid one-adult/one-minor transportation whenever possible

7.4 Prohibited Transportation Conduct

Drivers may not transport youth while impaired, distracted by handheld device use, or in an unsafe vehicle.

7.5 Overnight Travel

For overnight or tournament travel, unrelated adults may not share sleeping accommodations with minor participants, and FSPAL’s two-adult supervision expectation remains in effect whenever possible.


8. Refund Policy

8.1 General Rule

Program fees are due at the time of registration unless a scholarship, payment arrangement, or other written approval is granted by FSPAL.

8.2 Program Cancellation

If FSPAL cancels a program, a participant will receive either:

  • A full refund, or

  • At FSPAL’s discretion and with parent or guardian agreement, a credit toward another program

8.3 Withdrawal Before Program Start

If a participant withdraws before the program begins, a full refund may be issued minus any clearly disclosed administrative, processing, uniform-order, or nonrecoverable third-party fees already incurred by FSPAL.

8.4 Withdrawal After Program Start

If a participant withdraws after the program begins, refunds are not guaranteed and will be reviewed on a case-by-case basis depending on timing, expenses already incurred, roster impact, uniform or equipment orders, and the reason for withdrawal.

8.5 Medical Withdrawal

For medical withdrawals, a partial refund or program credit may be considered upon written request with supporting medical documentation.

8.6 Non-Refundable Circumstances

No refund ordinarily will be issued for:

  • Failure to attend

  • Voluntary withdrawal after completion of the first week

  • Disciplinary suspension or removal

  • Amounts already committed by FSPAL to leagues, officials, uniforms, insurance, or other non-refundable program costs

8.7 Refund Processing

Refunds will not be issued in cash. Approved refunds will be processed by check or approved electronic payment method, generally within 10 to 14 business days.


9. Concussion Management Policy

9.1 Requirement

Pursuant to Florida Statute § 943.0438, FSPAL requires annual parent or guardian informed consent regarding the nature and risk of concussion and head injury before a youth may participate in athletic competition, practice, tryouts, workouts, or related physical activity.

9.2 Removal and Return-to-Play

Any athlete suspected of sustaining a concussion or head injury must:

  • Be immediately removed from the activity

  • Not return the same day

  • Provide written medical clearance from an appropriate health care practitioner trained in concussion evaluation and management before returning to practice or competition

9.3 Guiding Principle

When in doubt, sit the athlete out.


10. Heat Illness Prevention

Florida conditions create significant heat-related risks for youth athletes. Coaches and program leaders must provide hydration opportunities, allow rest breaks, monitor for signs of heat illness, reduce exertion during dangerous conditions, and seek prompt medical care when symptoms warrant. (We cover heat related injuries for our staff during our First Aid/CPR/AED training.)


11. Gym Attire Policy

11.1 Purpose

To maintain a safe, respectful, and positive training environment, all participants using the Flagler Sheriff’s Police Athletic League gym must follow the attire and safety guidelines below.

11.2 Shorts / Bottoms

  • Shorts or bike shorts must have a minimum inseam of five (5) inches

  • Shorts must be worn at the waist and properly secured at all times

  • Clothing that is excessively tight, revealing, or restrictive is not permitted

11.3 Shirts / Tops

  • T-shirts must be worn at all times while using the gym

  • Shirts must fully cover the abdominal area

  • Shirts may not be excessively long or excessively tight

  • Shirts must maintain the original neckline and may not be cut, altered, or modified

11.4 Footwear

  • Closed-toe athletic shoes are required at all times while using the gym

  • Shoes must be securely fastened and appropriate for physical activity

The following footwear is not permitted:

  • Sandals

  • Slides

  • Flip-flops

  • Crocs or similar open-back footwear

  • Bare feet

11.5 Hats and Headwear

  • Hats, caps, or headwear must be worn properly and facing forward while inside the facility

  • Headwear that obstructs vision, contains offensive graphics, or presents a safety concern may not be worn during activities

11.6 Jewelry and Accessories

  • Excessive jewelry should not be worn during physical activity

  • Items such as large chains, hoop earrings, or loose accessories may pose a safety risk and may be required to be removed before participation

  • Removal is mandatory for combat sports when applicable

  • Staff may request removal of any item that creates a safety concern

11.7 Compliance

Participants who are not in proper attire or who fail to follow safety guidelines will be required to:

  • Change into appropriate clothing, or

  • Leave the facility for the day if proper attire is not available

Repeated violations may result in temporary suspension of gym privileges.


12. General Safety and Conduct Expectations

12.1 Expectations

All participants, spectators, parents, and staff are expected to:

  • Follow all staff and coach instructions and gym rules

  • Refrain from arguing with staff or coaches

  • Use equipment only for its intended purpose

  • Avoid horseplay, roughhousing, or unsafe behavior

  • Respect the facility, equipment, staff, spectators, and fellow participants

  • Remain in designated spectator areas and out of the field of play

  • Refrain from running around the gym during FSPAL programs

  • Clean up after themselves

12.2 Participant Discipline

Participants who violate these safety and conduct expectations may be subject to, in no particular order:

  • Verbal warning

  • Removal from play for a specified period

  • Temporary suspension from FSPAL programs

  • Removal from FSPAL programs

12.3 Spectator Discipline

Spectators, parents, or siblings who violate these expectations may be subject to:

  • Verbal warning

  • Being asked to leave the facility

  • Trespass from the facility during FSPAL programs


13. Alcohol, Drugs, Tobacco, and Vaping

Alcohol, illegal drugs, marijuana, tobacco products, nicotine products, electronic cigarettes, and vaping devices are prohibited at FSPAL facilities, youth activities, practices, games, meetings, and events.

No coach, volunteer, staff member, or participant may attend or participate while impaired.


14. Sponsorships and Donations

14.1 Definitions

A donation is a charitable gift made without a promised advertising, promotional, or other material return benefit. A sponsorship may involve marketing, recognition, advertising, or another return benefit to the supporter.

14.2 Compliance

FSPAL will not misrepresent a sponsorship as a fully deductible donation. FSPAL will issue acknowledgments and disclosures consistent with IRS rules, including quid pro quo disclosure requirements when applicable.

14.3 Restricted Use

No funds or donated items may be diverted from the represented program purpose without FSPAL approval.


15. FSPAL Youth Sports Athlete Evaluation Policy

15.1 Purpose

The purpose of athlete evaluations is to ensure fair team placement, balanced competition, and a positive developmental experience for all participants in FSPAL youth sports programs.

15.2 Scope

This policy applies to all youth athletes participating in FSPAL recreational sports leagues where player evaluations or drafts are conducted.

15.3 Evaluation Requirement

Newly registered athletes must attend the scheduled evaluation at the beginning of a new session. Evaluations allow coaches and program staff to assess each participant’s skill level and place them appropriately.

Athletes who wish to move to a higher level of play may be evaluated at the end of each session according to FSPAL’s current evaluation standards. If the athlete achieves a qualifying score, the athlete may be advanced to the next level of play.

15.4 Evaluation Process

During the evaluation session, athletes will participate in sport-specific skill drills, which may include:

  • Basic skill drills such as passing, catching, shooting, and dribbling

  • Agility and movement drills

  • Game simulation activities

  • Coach observation and scoring

Each athlete will be assigned an evaluation score based on performance, effort, and overall skill level.

15.5 Skill Level Classification

Athletes may be categorized into the following skill levels, when applicable:

  • Beginner: New participants or those developing fundamental skills

  • Intermediate: Participants with basic skill proficiency and some experience

  • Advanced: Participants demonstrating strong skills, game awareness, and experience

These classifications help promote competitive balance and appropriate instruction.

15.6 Evaluation Recording

Evaluation scores will be recorded on official FSPAL athlete evaluation forms. Information collected may include:

  • Roster number

  • Athlete name

  • Evaluation score

  • Shirt size

  • Shirt cut (male/female)

Evaluation forms will be reviewed and approved by designated program staff.

15.7 Team Draft and Placement

Following evaluations, teams may be formed using one or more of the following methods:

  • Coach draft

  • Staff-assigned balanced teams

  • A combination of evaluation scores and age grouping

The goal of team formation is to maintain competitive balance and fairness across all teams.

15.8 Staff Oversight

The evaluation process must be supervised by authorized FSPAL personnel. Evaluation forms must be approved by program leadership before teams are finalized.

Approved staff may include:

  • FSPAL Director

  • FSPAL Coordinator

15.9 Fairness and Conduct

All evaluations must be conducted in a professional, respectful, and unbiased manner. Coaches and evaluators are expected to:

  • Treat all athletes equally

  • Avoid favoritism

  • Focus on development and sportsmanship

Any concerns regarding the evaluation process should be reported to FSPAL program leadership.

15.10 Record Retention

Evaluation forms and draft records will be maintained by FSPAL for program administration and future reference during the season.


16. Media Release and Photography Policy

16.1 Authorized Use

FSPAL may photograph, video record, or otherwise capture images of participants during programs, practices, games, events, and activities for promotional, educational, and program-related purposes.

16.2 Consent

By signing the FSPAL registration, waiver, or policy acknowledgment, the participant’s parent or legal guardian grants permission for the Flagler Sheriff’s Police Athletic League to use such images and recordings in:

  • Social media platforms

  • The FSPAL website

  • Printed materials, flyers, and promotional content

  • Program-related publications and communications

16.3 Privacy

FSPAL will not publish or share personal identifying information of minor participants, including full names, addresses, or other sensitive details, in connection with any images or recordings.

16.4 Standards for Use

All media will be used in a manner that reflects the mission, values, and integrity of FSPAL programs.

16.5 Opt-Out

Participation in media is voluntary. Parents or guardians who do not wish for their child to be photographed or recorded must notify FSPAL in writing prior to participation. FSPAL will make reasonable efforts to honor such requests.


17. Policy Acknowledgment

All participants, parents, coaches, volunteers, and staff must review and acknowledge applicable FSPAL policies before participating in any program. Electronic acknowledgment through Gymdesk must be completed during registration.

YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU

ARE AGREEING THAT, EVEN IF THE FLAGLER SHERIFF’S POLICE ATHLETIC LEAGUE FLAGLER COUNTY SHERIFF ' S OFFICE, ITS DEPARTMENTS, EMPLOYEES, OFFICIALS, COACHES, CONTRACTORS, VOLUNTEERS, SPECIALISTS, AND AGENTS (HEREINAFTER REFERRED TO AS ( "RELEASED PARTIES") USE REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS

INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY

SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD'S RIGHT AND YOUR RIGHT TO RECOVER FROM ANY OR ALL OF THE RELEASED PARTIES IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND THE RELEASED PARTIES HAVE THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN THIS FORM.

I certify that I am the parent or legal guardian of the child listed in this registration and I am a person with the lawful authority to sign this document on behalf of the child listed below.

I certify that I have read or had this document read to me, that I understand its terms and

conditions and that I agree to abide by them.

I understand that my child will attend six classes and may have to preform physical labor to include but not limited to cleaning and picking up trash in local communities.

I certify that I understand the nature of the S.W.E.A.T. Program and feel comfortable regarding what my child will be doing as a participant.

I understand that I am responsible for providing written notification of any accommodations

needed due to medical, mental or physical limitations, disabilities or conditions that limit the

participation of the minor listed below in these activities.

I understand the Released Parties reserve the right to disallow or terminate participation of the minor if it would be unsafe for the minor to participate in the S . W.E.A.T. Program.

I hereby give permission for my child to receive necessary emergency medical treatment and

transportation to a medical facility in the event of a medical emergency. I acknowledge I am

responsible for payment of said services.

I understand the Released Parties are not responsible for any property that is lost or destroyed

while participating in the S.W.E.A.T. Program.

I understand that my child may be removed from the program at any time if my child is

disruptive, conducts him or herself in a manner that is unsafe, violates the rules and regulations

of the program and/or refuses to participate and follow directions.

·        No smoking or tobacco products of any kind

·        No horseplay or use of obscene language

·        No cell phones, ear buds or headphones, or electronic devices

·        No loud talking, talking during presentations, gesturing, or shouting

·        No throwing of trash or other articles

·        You must always participate and be respectful

I understand that to participate my child must be properly attired for the activities of the

day including comfortable and safe footwear, preferably sneakers. I must dress my child in

weather-appropriate clothing. clothing that is not allowed includes but is not limited to, clothing that does not cover the entire torso, hats, shorts with less than a 5” inseam, tight fitting clothing, clothing with images or messages that do not communicate a positive message. Hooded sweatshirts are allowed but the hood cannot be worn during SWEAT. Youth wearing clothing that does not conform to the clothing standard will not be allowed to participate.

I agree that in consideration for my child's participation in the Flagler Sheriff’s PAL INC and the Flagler County Sheriff’s Office S.W.E.A.T. program that I shall hold harmless and fully indemnify and defend the Released Parties from any and all causes of action, claims, damages, costs, including by not limited to attorney's fees and costs which may arise any cause of action by me or by, through or on behalf of my child, even if the damages, injuries or death are caused in whole or in part by the

negligence of the Released Parties.

I expressly agree this Release and Waiver of Liability Agreement is intended to be as broad and inclusive as is permitted by the Laws of Florida and that if any portion is held invalid. I agree that the balance shall notwithstanding. continue in full legal force and effect.
